About the panelists:
Melissa Sheldrick is a trained educator and has been working with families for more than 20 years. She now works as Patient and Family Advisor at ISMP Canada and began advocating for improved safety after the loss of her 8-year-old son, Andrew, from a medication error in 2016. Since that time, Melissa has been working with organizations to advance continuous quality improvement programs in community pharmacy, including error reporting. She is an active member of Patients for Patient Safety Canada and continues to advocate for patient safety, and more specifically medication safety, across the country and internationally.
Dr. Dalyce Zuk received her Bachelor of Science in Pharmacy Degree from the University of Alberta in 2005 then went onto complete an Accredited Clinical Pharmacy Residency with the former Capital Health Region. She completed her Doctor of Pharmacy Program in 2008 at the University of British Columbia, taking specialty electives in solid organ transplantation at the University of Cincinnati and University of Alberta. She returned to the University of Alberta and completed a Clinical Pharmacotherapy Practice Fellowship in Solid Organ Transplant in 2010. She was a Clinical Practice Manager with Provincial Pharmacy Services for ten years, where she was responsible for providing leadership, direction, and guidance over Pharmacist Practice and standards, in support of the vision, mission and business plan of Alberta Health Services. In 2021, she became the Clinical Content, Compounding and Repackaging Manager within Provincial Pharmacy Services. She leads a team of 13 pharmacists and pharmacy technicians, responsible for the standardization, creation and maintenance of sterile, non-sterile, and repackaging recipes/worksheets within Alberta Health Services. These worksheets serve as the source of truth for build in Connect Care, the electronic prescriber order entry system being rolled out across the province of Alberta.
Tracy Gallina is the Clinical Director, Product Quality for HealthPRO Procurement Services, overseeing the product evaluation and product concerns processes for the Pharmacy contracting portfolio. Tracy is passionate about contributing to patient safety through her role at HealthPRO. Working with the Product Evaluation Committee, she applies her knowledge and experience to advocate for product improvements to support medication safety in hospitals by sharing identified opportunities for improvement with suppliers. Tracy is a registered pharmacist with 30 years’ experience in both community and hospital pharmacy settings. She holds a Bachelor of Science in Pharmacy from the University of Toronto and a BA in Psychology from McMaster University and has also completed a specialty pharmacy residency in Cardiology and Critical Care.
