Do you know a Pharmacy Technician or Assistant that goes above and beyond the regular call of duty to provide outstanding patient or customer care? Let them know how much they are appreciated by nominating them as CAPT’s Pharmacy Technician or Assistant of the Year.
Please provide a summary that outlines how the nominee has served the health care industry in Canada. This may include, but is not limited to, contributions to community service and employment. It is appropriate to note contributions on a local, provincial, national, or international level here as well. Clearly state the roles and responsibilities for each service, and what achievements were experienced because of demonstrated excellence in patient care, leadership and/or professionalism. Also identify awards, special recognition and honors received in the past. Limit summary to two pages including
the CAPT nomination form. Please reverse chronological order to present activities and define acronyms. Include specific names of businesses and organizations, and dates of involvement.
Each nomination must be accompanied by two references that provide insight about the attributes, contributions, and achievements of the candidate.
Applications will be adjudicated by the Board of Directors of the Canadian Association of Pharmacy Technicians. Adjudicators will evaluate the nomination and references in context with each other, 75% weighting on the nomination and 25% on the references. Emphasis will be placed on the achievements of each candidate. CAPT will request photo headshots from the selected finalists and may request additional information. CAPT reserves the right to verify any information provided by the finalists. By allowing oneself to be nominated, the pharmacy technician or assistant is agreeing to allow his or her information to be considered as potential CAPT marketing or editorial content. Decisions of the adjudication committee will be final.
Please submit nominations by March 28, 2020. The winner will be announced at CAPT’s annual Professional Development Conference.
